New Patients
During your first visit, please arrive early to complete paperwork and bring the following:
- New Patient Forms (Below)
- Identification Card
- Patient's Insurance Card
- List of current prescriptions and over-the-counter medications
- Medical History Information
You are responsible for co-payments or charges that are not covered by your insurance. If you have questions regarding billing or which insurance plans we accept, please call our office at (941) 929-1234. If you have questions regarding your insurance coverage and benefits, they should be directed to your employer or insurance company.
